Values clarification, surrogate designation, living will. Most comprehensive engagement.
Ongoing Care Coordination Retainer
Monthly ongoing coordination. Scope and included hours defined at intake.
Second Opinion Coordination
Identifying the right specialist, records coordination, preparation, and post-consultation debrief.
Insurance Navigation & Appeals Support
Review of denial, appeal preparation, physician coordination. Minimum 2-hour engagement.
Advance Care Planning (Document Review 1–2 sessions)
Review of existing documents with findings, recommendations, and one follow-up session.
Family Consultation
Single session for a specific question or decision. Includes written summary.
Facility Research & Placement Support
Research, evaluation, schedule tours, placement coordination.
Crisis Navigation (Urgent, Same-Day)
Premium rate for urgent same-day or next-day engagement.
Special Needs & Complex Pediatric Care Planning
For a specific question, a single issue, or a one-time need IEP meeting preparation, a denied therapy authorization, help understanding a new diagnosis. Other comprehensive packages available upon request.
Hospital & Care Transition Navigation
Active coordination through discharge or transition. Typically 4–10 hours per engagement.
Family Consultation Extended (Mediation)
Structured facilitation for family conflict around care decisions. 90 minutes to 2 hours.
FMLA Navigation
This 90-minute session is built for working adults and family caregivers who need expert guidance not legal advice, but clear, accurate navigation of a complex process before they walk into HR or pick up the phone.
Aging & Long-Distance Care Planning
Structured planning session(s) for remote caregivers. Retainer option for ongoing oversight
