ABOUT US
Landmark Care Navigation was not born in a boardroom. It was born in a hospital hallway, in the middle of grief. When my grandmother was in hospice with a terminal diagnosis, my uncle and I noticed my aunt could not recall anyone’s name. Worried, we took her to the hospital. I knew she needed specific diagnostic tests ordered, and the system was moving right past her. Because I was there, because I knew the language, the process, and the pressure points, those tests got ordered. They revealed she had suffered a stroke. Without someone in that room who understood how to navigate the system, that diagnosis would have been missed. That moment made something undeniable: most families do not have a clinician in the room. And the consequences of that absence are not equal for everyone.
As a Licensed Clinical Social Worker with over a decade of experience, I have sat beside patients at every stage of the healthcare journey, from diagnosis to discharge, from insurance battles to end-of-life transitions. And when I kept watching the system fail the people I served, not because care did not exist but because policy and law were blocking access to it, I went further. I earned a Master of Jurisprudence in Health Care Law from the University of Washington to understand the legal architecture of healthcare. Then I went to Capitol Hill, spending a year in Congress learning firsthand how legislation is made and how the laws that govern access to care are written, debated, and passed. I did not just study the system. I went inside it.
That depth of understanding is what Landmark Care Navigation is built on. We exist to be the advocate that every family deserves but most never have, helping clients navigate insurance and healthcare systems, coordinate care across providers and specialists, and support aging individuals, mothers, and families through the most critical health transitions of their lives.

Meet The Founder
LaCinda Jones, LCSW, MSW, MJ, PMP
With over a decade of clinical social work experience, LaCinda has worked at the bedside in intensive care units, sat with families in the hardest moments of their lives, and advocated for patients in rooms where the power dynamic was rarely in their favor. She knows what it means to be the person in the room who understands what the doctors are saying and what it costs a family when that person isn’t there.
That firsthand experience drove her further. She earned a Master of Jurisprudence in Health Care Law to understand the legal architecture that governs access to care. She then spent a year on Capitol Hill as a Congressional fellow, learning how the laws that shape healthcare are written, debated, and passed. She has since served in federal health policy, giving her a perspective that spans the clinical, the legal, and the legislative.
LaCinda is a Licensed Clinical Social Worker, a certified Project Management Professional (PMP), and a trained change management practitioner. She is a member of the Council on Social Work Education Board and brings that same commitment to standards and ethics to every client engagement.
What she offers is not just coordination. It is advocacy grounded in clinical expertise, informed by law and policy, and delivered with the kind of presence that helps families move from overwhelmed to clear.
